This video series describes successful linkages among primary care, public health, and clinical community resources in the state of Vermont. Vermont’s Blueprint for Health program provides comprehensive, coordinated care while improving health outcomes and reducing costs.

The third module establishes the Blueprint for Health as a generalizable, scalable, and sustainable model and discusses key principles as they relate to adoption by others.
The first module introduces the Blueprint for Health–a Vermont statewide public-private initiative that seeks to provide incentives and infrastructure to support the delivery of coordinated, high quality health services. A patient’s story illustrates this coordinated care.
The second module demonstrates the creation of Patient Centered Medical Homes that are supported by Community Health Teams working in conjunction with public health services to improve the health of individuals and populations. It reviews the incentive structure for health care providers and outlines multiple levels of the Community Health Teams (i.e., core, extended, and functional).
The Heart 360® Innovation was one of several programs featured in a Million HeartsTM event in April 2012. The Million HeartsTM initiative invited prominent health care thought leaders and stakeholders to Washington DC, to inspire creative thinking about scaling and spreading cardiovascular prevention activities throughout the United States. Drs. Carolyn Clancy, Director of the Agency for Healthcare Research and Quality and Thomas Frieden, Director of the Centers for Disease Control and Prevention, inspired and motivated attendees to achieve the Million HeartsTM goal. Throughout the day the attendees shared real-world success stories about service delivery innovations shown to improve blood pressure and cholesterol control. Attendees also brainstormed ideas to scale and spread these and other innovative approaches to better heart health.
