The tool uses information such as blood pressure, smoking status, diet, and physical activity to determine the user’s current heart score. It also provides a customized action plan to help users improve their score and reach their ideal cardiovascular health.

Advancing Pharmacy Health Literacy Practices Through Quality Improvement: Curricular Modules for Faculty 03/13/12

These curricular modules, for pharmacy faculty, integrate health literacy and health literacy quality improvement into courses, experiential education, and projects for PharmD students and pharmacy residents. The modules are adaptable for use in lectures, seminars, laboratory classes, and experiential education.

Heart Attack Risk Calculator 01/13/12

This risk assessment tool, for people age 20 or older, provides information to assess a person’s risk for heart disease, heart attack, or other future heart problems. The tool uses information such as blood pressure, cholesterol levels, and blood sugar levels to help determine a level of risk for heart problems. Upon completing the questionnaire, users can print various reports to help them make a plan with their doctor to reduce their risk.
