The Collaboration for Home Care Advances in Management and Practice (CHAMP) library provides actionable tools to help home health care professionals implement best practices in their agencies.
Tools in the CHAMP library can be searched by keyword and filtered by topic or type:

This toolkit, developed by the Center for Health Care Strategies (CHCS), can help guide efforts to improve care for children in the child welfare system. The toolkit presents the work of 9 managed care organizations that participated in a 3-year CHCS quality improvement collaborative. It provides details about the pilot efforts of each plan, including identification, stratification and outreach strategies developed, as well as obstacles faced and preliminary outcomes achieved.
This guide was created to propel long-term care communities in their improvement efforts by presenting a collection of concrete strategies for actualizing a resident-directed, relationship-centered philosophy. It supplies providers with tools, data, and practical resources so they can make informed decisions as they consider implementing culture change initiatives to deliver person-centered care.

This guide, for hospital administrators and emergency department leaders, provides step-by-step instructions for planning and implementing patient flow improvement strategies to ease emergency department crowding.

These curricular modules, for pharmacy faculty, integrate health literacy and health literacy quality improvement into courses, experiential education, and projects for PharmD students and pharmacy residents. The modules are adaptable for use in lectures, seminars, laboratory classes, and experiential education.
The modules consist of 17 activity guides with 4 accompanying PowerPoint presentations. Each activity guide includes a list of further resources.
