This toolkit can help emergency medical services (EMS) agencies develop and operate Mobile Integrated Healthcare and Community Paramedicine (MIH-CP) programs. The toolkit contains samples of forms, documents, and questionnaires that EMS agencies are currently using to run their MIH-CP programs.
MIH-CP is the provision of health care using patient-centered, mobile resources in the out-of-hospital environment. It may include, but is not limited to, services such as:
This toolkit provides resources and recommendations for reducing inappropriate emergency department (ED) use. It includes key lessons to begin the process of reducing ED use, information on how data can help to identify avoidable ED use, and initiatives currently in place to engage stakeholders. A downloadable resource guide provides additional materials for EDs to use when undertaking this initiative, and a video explains the appropriate use of ED care.
The Conversation Starter Kit can help consumers organize their thoughts and wishes, and then have conversations about end-of-life care with their friends, family, and doctor. The kit gives suggestions on steps to take while making these decisions. There is also a form where individuals can put their end-of-life decisions onto paper.
Advance Care Planning is a way to help you think about talk about and document wishes for health care. A Goals of Care Designation is a medical order used to describe and communicate the general aim or focus of care including the preferred location of that care. Although Advance Care Planning conversations don’t always result in determining a Goals of Care Designation they are useful building blocks to conversations.
