The Safety Reporting Portal streamlines the process of reporting product safety issues to the U.S. Food and Drug Administration (FDA) and the National Institutes of Health (NIH). The portal allows the public (manufacturer, health care professional, researcher, public health official, or concerned citizen) to submit a safety report about the food supply, medicines, and other products in the United States.

Reports can be submitted for safety issues involving the following:

  • Human or animal reportable foods
  • Animal drugs
  • Pet foods
  • NIH gene-transfer research
Registration is optional, but is required to save reports.
