This toolkit provides strategies and templates to help health care professionals reduce the incidence of vaccine-preventable pneumonia and related morbidity and mortality in senior housing settings. The goal of this quality improvement project is to increase awareness and rates of pneumococcal vaccination among residents and staff in these settings.

The toolkit includes the following resources:

  • Algorithm and instructions for toolkit implementation
  • Sample data collection form and quality measures/calculation instructions
  • PowerPoint slides available for interventions
  • List of resources available for interventions
  • Sample action plan template
