This toolkit is designed for hospital and clinical directors, managers, clinicians, and researchers to assist in engaging patients as partners in quality improvement activities. It contains two tools that can be customized by organizations to match their goals, processes, and patient engagement strategies:

  • Patient Partners Welcome Packet: This packet is designed for patients who will be participating in quality improvement activities at a health care organization. It contains orientation information to help patients feel comfortable with participating in meetings and quality improvement activities.
  • Patient Engagement Toolkit for Team Members: This packet is intended for health care employees who will be working with patients on quality improvement activities. It contains tools that team members will need to successfully engage patients.
