Participant Experience Survey (Brain Injury)

Description

This survey solicits feedback from program participants with brain injury about the services and supports they receive under the Medicaid Home and Community-Based Services waiver program. States can use these data to calculate performance indicators for monitoring quality within their waiver programs, to guide quality improvement projects, and to use in conjunction with other quality management strategies.

This survey addresses these priority areas:

  • Program supports
  • Choice and control
  • Respect and dignity
  • Community activities

The survey can be used to calculate performance indicators within these priority areas for the entire sample or for different subsamples for comparison across groups.

The survey also includes questions on the following topics:

  • Community integration
  • Activities of daily living/independent activities of daily living
