This tool is designed to help employers assess the extent to which they have applied evidence-based health promotion interventions in their worksites to prevent heart disease, stroke, and related conditions such as hypertension, diabetes, and obesity. The CDC Health ScoreCard can establish benchmarks and track improvements (evaluation) over time for various health topics and promotion programs.

The tool contains 100 questions that assess how evidence-based health promotion strategies are implemented at a worksite. Employers can use this tool to assess how a comprehensive health promotion and disease prevention program is offered to their employees, to help identify program gaps, and to prioritize across the following health topics:

  • Organizational supports
  • Tobacco control
  • Nutrition
  • Physical activity
  • Weight management
  • Stress management
  • Depression
  • High blood pressure
  • High cholesterol
  • Diabetes
  • Signs and symptoms of heart attack and stroke
  • Emergency response to heart attack and stroke
