Building Your Medical Home


This toolkit for health care professionals helps develop a pediatric medical home and improve patient care. The elements in the toolkit help to increase outcomes, such as health status, timeliness of care, family centeredness, and family functioning in a medical home. Achieving the standards in the toolkit is a requirement to be in the National Committee for Quality Assurance Physician Practice Connections (r) Patient Centered Medical Home Recognition program.

The toolkit is organized into six areas that present tools for medical home implementation.
