Improving Medication Safety in High Risk Medicare Beneficiaries Toolkit

Description

This toolkit is based on experience gained in the Medication Evaluation and Drug Use Problem Identification to Improve Safety in High Risk Medicare Beneficiaries (MEDIS-MB) study. The toolkit has two goals: to describe the medication therapy management (MTM) program assessed in the MEDIS-MB study, and to provide experience and tools used in the assessment. Two groups can benefit from using the toolkit: researchers who are evaluating MTM programs, and MTM service providers who need tools for documenting these services.

The toolkit assists in the following aspects of preparing an MTM program:
  • Develop and conduct an MTM program
    • Describe the development and goals of the MEDIS-MB study
    • Describe the MEDIS-MB program
    • Make clinical MTM tools developed by the MEDIS-MB program research team available to others
  • Assess an MTM program
    • Describe the strengths and limitations of the tools used in the assessment of the MEDIS-MB program
    • Make the MTM assessment tools available to others
