Stroke Toolkit


This toolkit can guide physicians in the acute management of stroke patients. The kit includes forms and documentation to assist in critical decisionmaking. Designated forms that must accompany the patient upon transfer are identified.

The toolkit includes:

  • Stroke packet cover
  • Possible IV recombinant tissue plasminogen activator (rt-PA) (Activase) candidate (inside 3-hour window or less than 180 minutes from symptom onset)
  • IV t-PA inclusion/exclusion checklist
  • 0.9 mg/kg rt-PA dosing chart
  • Blood pressure management, rt-PA
  • Consent form for rt-PA
  • Reference sheet for MDs–Risk/benefit of rt-PA: Facts for patient and family
  • Consider intra-arterial thrombolysis
  • Other considerations: Endovascular intervention
