Description
This toolkit and self-assessment tool help health care organizations develop a culture that supports respect and effective communication with patients and families around adverse events. The Disclosure Toolkit provides selected tools, literature, and other resources to help establish an environment that supports effective communication with patients and families. The Disclosure Culture Assessment Tool enables organizations to determine their current cultural environment and structures needed around safety and disclosure of adverse events.
