Health ShackTM


Description

Health Shack assists in the development of independent and emancipated youth by providing an electronic Personal Health Information System (PHIS) designed for and with these youth. It offers informational resources, as well as safe keeping and accessibility for important life documents.

This service allows its members to download and store important health information, so that when a member or a care provider needs the information, they can get it quickly. Health Shack members can choose to share their information with other people such as their medical providers, family members, and advocates. Health Shack also includes an electronic medical record (EMR) "lite" so that medical providers can, with permission, enter and/or update health information.

Types of information that Health Shack can store include:
  • Medical history
  • Names of doctors and other care providers
  • Immunization records
  • Hospital visits
  • Insurance, address, and emergency contact information
