Description
The Health Care Coordination Toolkit is designed to assist individuals, guardians, and teams in the proper coordination of health care for adults with developmental disabilities. It includes forms to guide the information that should be tracked for effective care coordination.
The intent of the toolkit is to assure that individuals receive timely medical followup, testing, and monitoring for adequate health care delivery.
The materials are organized into the following sections:
