Establishing Partnerships Between Managed Care and Aging Service Organizations


Description

Health care organizations are being challenged to care for the growing number of older adults with chronic health conditions. This manual describes how to establish partnerships between managed care and aging service organizations to assess and improve the quality of care provided to people with dementia. It is based on the experience of the Alzheimer's Association-Kaiser Permanente Metropolitan Los Angeles Dementia Care Project.
