Description

This wallet card provides space to help people with diabetes maintain records of medications and medical tests, identifies personal targets for preventive medical care, and provides information that can be used by others in case of emergency.

This wallet card allows users to record the following information:

  • Medications, dosages, and times
  • Name
  • Allergies
  • Names and telephone numbers for the doctor, other health care providers, and an emergency contact
  • Targets (goals) and dates for preventive health care services

The tool is available in English, Portuguese, and Spanish.
